RealSelf Releases its Spring 2025 Real Talk Report on 5 Aesthetic Trends to Watch

Leading Plastic Surgeons Across the U.S. Predict the Most-Requested Treatments Will Focus on Natural-Looking Results

RealSelf, the largest and most trusted online resource for people exploring cosmetic treatments and providers, has released its quarterly Real Talk Report, offering a comprehensive look at five key trends shaping the aesthetics landscape for Spring 2025.

The report combines data from millions of consumer searches on RealSelf with in-office insights from 17 innovative plastic surgeons and dermatologists practicing across the U.S.—from Beverly Hills and Manhattan to rising aesthetics hotspots like Atlanta and Austin.

Top trends and takeaways:

  • Regenerative treatments are the future of aesthetics, as patients look for a more holistic and long-term approach to facial rejuvenation, according to many of the providers who weighed in. “There’s a clear trend toward improving skin quality rather than relying solely on fillers and neurotoxins,” notes Dr. Goretti Ho Taghva, a board-certified plastic surgeon in Newport Beach. As patients seek more natural solutions for radiant skin, a growing number of practices offer proprietary treatments that harness the restorative benefits of PRF, exosomes, and PDRN derived from salmon sperm—often in combination with microneedling. Dr. H. William Song, a physician in Oakland, NJ, has even seen renewed interest in classic treatments like the Vampire Facelift, which combines fillers with PRP.
  • Refreshed and defined eyelids will also be a standout spring trend as interest in upper and lower blepharoplasty grows across all groups, including more male patients. RealSelf saw a 324% year-over-year increase in searches for eyelid surgery last spring, and it’s the #1 most-searched procedure so far in 2025. Surgeons consulted for this report say patients are seeking a refreshed look, without altering their natural eye shape. Upper blepharoplasty will especially be popular among women who are mindful of evolving makeup trends that emphasize a defined eyelid contour. To correct lower eyelid hollowness or bags, board-certified facial plastic surgeon Dr. Dilip Madnani in New York City predicts more people shying away from under-eye fillers in favor of “more permanent results with surgery.”
  • Surgical facial rejuvenation is one of the biggest trends predicted for this spring, as more people seek out deep plane facelifts or mini lifts to enhance their natural beauty. Facelifts are the third most-searched procedure on RealSelf so far this year, and facial plastic surgeons predict its popularity will continue to rise as more people realize results from modern facelift techniques look more natural than fillers, with far more longevity. Several surgeons consulted for the report say more patients are combining facelifts and neck contouring with complementary procedures like lip lifts, fat transfer, and chin liposuction to sculpt and rejuvenate the entire face and neck. “This approach allows for more balanced, natural-looking transformations while minimizing overall downtime,” explains Dr. Shahram Salemy, a board-certified plastic surgeon in Seattle.
  • Downsized breasts and “internal bras” will also be a common patient request this spring, as patients look for enhanced support and comfort, lower implant complication rates, and perkier, more naturally shaped breasts that look proportionate with their frame. Dr. Urmen Desai, a board-certified plastic surgeon in Beverly Hills, says the desire to downsize reflects both celebrity-driven trends and an increased focus on fitness, wellness, and long-term health.
  • Body contouring procedures will also surge as we approach summer. Several plastic surgeons cited tummy tucks, body lifts, brachioplasties (arm lifts), thigh lifts, and full mommy makeovers among their most-requested procedures as people anticipate warmer weather. Dr. Suzette Miranda, a board-certified plastic surgeon in Seattle, also points to the exploding popularity of GLP-1s as driving the demand in surgical body contouring, as more patients seek solutions for excess skin left by rapid weight loss.

“In a world where the notion of beauty is always changing, anticipating trends and understanding how they help inform aesthetic decisions for the average consumer is key,” said Minou Clark, CEO of RealSelf. “As a legacy brand, it’s important we stay one step ahead as we work with providers to further destigmatize cosmetic procedures. We have a unique window into consumer behavior on RealSelf. Hearing from so many plastic surgeons and other leading aesthetic physicians about what they're seeing in their practices gives us an even more complete picture.”
